CTA Freedom Parent Volunteer Agreement

We are so thankful you have decided to become one of our parent volunteers! There are many different ways you can be helpful in the classroom and during special events. Thank you for getting involved!

I understand that each time I volunteer I must sign in at the front office upon arriving and leaving campus.

I understand that I should always have a visitor sticker where it is visible, on the front of my shirt.

I understand that the teacher may assign me tasks based on his/her needs, and that these tasks may vary.

I understand that I need to pre-arrange my volunteer time and location with the teacher.

I understand that if I am unable to make a scheduled volunteer time, I will notify the teacher ASAP.

I understand conversations with both teachers and students need to remain professional.

I understand I’m not allowed to have children accompany me while volunteering at CTA Freedom. Volunteers must be 18 years of age or older.

I understand that as a volunteer I may be privy to private or sensitive information about student learners and agree to keep all information, conversation, and knowledge learned confidential.

I understand that I may be subjected to a background check in order to volunteer at CTA Freedom.

I understand that failing to comply with this agreement will cause revocation of my ability to volunteer on campus.

On behalf of the Governing Board and Administration of Chandler Unified School District, we appreciate your interest in our district. We are proud of our outstanding teachers and support staff; however, the quality of our services is significantly enhanced by hundreds of individuals, like you, who contribute their time and talents.

Because of the tremendous responsibility we have to the children of our community, the following information is needed from each individual who has contact with our students. The district may choose to conduct a background check on individuals, including volunteers, who provide services to students.

Please be aware that the district may decline volunteer services based upon criminal activity related to violence, physical abuse, sexual abuse, or alcohol/drugs. We appreciate your willingness to provide us with this information.
